Background
An 82-year-old retired orthopedic specialist (referred to here as Mr. Li) presented with persistent painless gross hematuria — visible blood in the urine without discomfort during urination. Despite multiple imaging studies at another hospital, including urinary tract ultrasound, CT urography, pelvic MRI, and whole-body PET-CT, the primary tumor source could not be conclusively identified.
Imaging showed abnormal hypermetabolic lesions in the liver, skeleton, and multiple abdominal/pelvic lymph node groups, together with elevated metabolic signal in the prostate region. The initial working diagnosis leaned toward prostate disease, and a systematic prostate biopsy was performed — but pathology found no malignant cells. Meanwhile, Mr. Li's hemoglobin declined progressively, his physical strength deteriorated, and the bleeding continued. The case reached a diagnostic impasse.

How MDT Re-evaluation Changed the Diagnosis
A multidisciplinary team (MDT) — bringing together urologic oncology, medical oncology, radiology, and pathology specialists — revisited the case from first principles. Instead of following the initial prostate-focused assumption, the team used the core clinical symptom — persistent painless gross hematuria — as the diagnostic anchor.
Repeat urinary tract ultrasound revealed a large blood clot (5–6 cm) inside the bladder obscuring the field of view, and a suspicious solid mass (~1.5 cm) on the posterior bladder wall. The team proceeded with cystoscopy and diagnostic transurethral resection to obtain tissue from the bladder lesion, alongside a percutaneous liver biopsy.
Pathology confirmed high-grade muscle-invasive urothelial carcinoma originating in the bladder, with synchronous liver metastases and diffuse bone metastases — a complete explanation for all prior abnormal imaging findings, the ongoing bleeding, and the moderate-to-severe anemia.
The Challenge: Standard Chemotherapy Was Not Suitable
For advanced urothelial carcinoma, cisplatin-based combination chemotherapy has long been the standard first-line regimen. However, at age 82 with declining physical reserve and progressive anemia, Mr. Li was not a candidate for platinum-based chemotherapy — the risk of severe bone-marrow suppression, gastrointestinal toxicity, and further functional decline was judged to be unacceptably high.
This is a common dilemma in geriatric oncology: the same treatment that controls cancer in a younger, fitter patient may cause more harm than good in a frail elder. The question becomes how to achieve meaningful disease control while preserving daily function and quality of life.
Personalized Low-Toxicity Approach: ADC Plus Immunotherapy
The MDT formulated an individualized plan centered on two classes of newer systemic agents:
- Antibody–drug conjugate (ADC): An antibody linked to a cytotoxic payload delivers chemotherapy directly to tumor cells expressing a specific surface marker, sparing much of the healthy tissue that would be affected by conventional intravenous chemotherapy.
- Immune checkpoint inhibitor (ICI): A class of drug that releases the brakes on the body's own immune system, enabling T cells to recognize and attack cancer cells more effectively.
In suitable patients, the ADC-plus-ICI combination has shown efficacy against both the primary bladder tumor and metastatic lesions, with a toxicity profile generally more favorable than traditional platinum doublets — making it a rational option when standard chemotherapy is contraindicated due to age or comorbidity.
Beyond the immediate regimen, comprehensive gene profiling (next-generation sequencing) was performed to map potential future targets. For patients with advanced metastatic disease, having molecular results on file means that if the current therapy loses effectiveness or becomes intolerable, subsequent targeted or immunotherapy options can be deployed without waiting for a new testing cycle.
Supportive Care Alongside Anticancer Therapy
Treatment for advanced cancer in elderly patients extends beyond choosing the right drugs. Equally important is proactive management of complications:
- Bleeding control: Local measures and systemic interventions to reduce hematuria and stabilize hemoglobin.
- Bone health: Bone-modifying agents and early intervention to lower the risk of skeletal-related events (pathological fractures, severe bone pain).
- Anemia management: Monitoring and correcting low hemoglobin to maintain energy levels and organ perfusion.
- Functional preservation: Maintaining mobility, nutrition, and activities of daily living throughout the treatment course.
After the first treatment cycle, Mr. Li's bleeding and fatigue improved noticeably, and overall disease status stabilized according to preset follow-up parameters. A comprehensive reassessment is planned at the three-month mark, at which point the MDT will review tumor response and tolerance before adjusting the ongoing plan.
